New Client Paperwork

If you are a new client there are several forms that you will need to complete prior to our first meeting.  You can download and print them at home and bring them with you.  The forms will also be available at my office before our scheduled appointment time.  Please arrive 10-15 minutes early if you plan to fill them out at my office, so that we can begin our session at the scheduled time.  If you have any questions, we can discuss them at our session. 

Telephone and Internet session

Once we have worked together for some time, and if circumstances demand (travel, etc), we may consider having sessions via telephone or Internet.  Payment for such sessions is based on the same fee.

Payment Options

Payment is due in full at the time of service (unless other arrangements are made between us).  I can accept payment in cash, check, or credit card.  A processing fee of $5.00 (per session fee) is added to any credit card payment.

My initial assessment and treatment plan involves 1 hour of administrative work, which I provide pro bono.  Ongoing administrative services associated with your treatment, such as site visits, report writing, record reading, consultation with other professionals, or telephone conversations (other than brief scheduling issues), will be prorated and billed at $150 per hour, unless agreed otherwise.  Brief email communications are provided pro bono.

Please note that I am not an in-network provider for any insurance carrier, except Aspire (CHOMP) through 2019.  Payment for all services is the financial responsibility of the client.  However, some insurance companies will pay for out-of-network services (PPO).  Clients should remember that my services are rendered and charged to the client, not an insurance carrier.  It is the client’s responsibility to know his/her insurance policy with regards to reimbursement for rendered services.  Please call your insurance carrier to inquire about mental health services.  I will be happy to provide you with a comprehensive statement (Superbill) that you can submit to your insurance carrier for possible reimbursement.  I can also provide you a document to advocate for mental health services if requested by you.  Please click HERE to learn more about insurance and coverage for mental health services.
